Many people don't know this, but a sinus is actually a hollow space or cavity filled with either blood or water. There are a number of sinuses in our entire body. Untreated sinus problems may lead to serious complications, like bone infections, or infections of the tissues around the eye, meningitis, or brain abscess. Chronic infections may also lead to chronic postnasal drip and congestion. Thus, it is recommended that people suffering from such chronic sinus problems also stay away from smoking and being exposed to too much pollutants to avoid triggering sinusitis.

A lot of people often dismiss headaches as simply nothing and just resort to analgesics and other kinds of painkillers to relieve the discomforts they feel. However, if the condition becomes chronic, or recurs, it should not be taken for granted, as it could be the onset of something grave, like a serious sinus problem.

Book Review of "Sinus Relief Now" by Dr. Jordan Josephson-Part 2

4. Regarding surgery, Dr. Josephson was a pioneer in the procedure called FESS (Functional Endoscopic Sinus Surgery) which is now considered state of the art. This procedure uses an endoscope for both a diagnosis as well as during the surgical procedure. This gives the surgeon an excellent view of the area being worked on, and thus there is less bleeding, more accuracy, and packing is rarely necessary, which is very important. He says many of his patients simply go home and go to work the next day. I would have liked him to comment about endoscopic surgery using lasers as well, but he didn?t make mention of that technique. People have told me that this laser procedure worked exceptionally well for them. Dr. Josephson is obviously totally committed to FESS, however, and with the micro tools he uses, he apparently is able to keep bleeding and pain levels low for patients, as is the case for endoscopic laser surgery. He also makes no comments about a new surgical procedure called sinuplasty, which is similar in concept to angioplasty. I would certainly like to hear his opinion of that new procedure.

5. He wrote at length about new research developments, especially the remarkable work done by Mayo Clinic researchers regarding the effects of fungus on sinusitis. Although the results are still somewhat controversial, this team has shown that an atypical immune response reaction to fungus in certain people to common fungus might be the root cause of sinusitis. The theory is growing in popularity and new topical anti-fungal medications are being developed.

7. He emphasizes that eating the right foods can make a big difference, but he isn?t totally dogmatic about it. I was turned off once by another book in which the author stated that one must simply give up certain foods if they wanted to get over sinusitis, and these included cheese, chocolate, wine and all other alcoholic beverages, among other things. For a person like me who lived for several years in France, this would be unbearable. I might mention that for a French person it would be unthinkable.

8. A clean living environment is also stressed, and the book has motivated me to break out the bleach bottle and give our bathroom the once over. Molds can apparently grow very rapidly on wet items, so one should be careful about having wet clothes or rags lying around, for example.

9. Also included in the book is a discussion about the validity of alternative therapies, like acupuncture, holistic medicine, and chiropractic, for example. I think this is useful, and in fact a niece recently told me she had acupuncture treatments for her sinus problems and she was excited about how much better she felt.

The five step plan that Dr. Josephson recommends is presented at the end of the book. The steps are:
1. Take care of your sinuses through proper irrigation.
2. Clean up your environment
3. Be vigilant about food choices
4. Take your medication (i.e., take the entire prescription)
5. Embrace life-altering changes and enjoy your health.

There has been a lot of progress made in the treatment of sinusitis in the past 5-10 years, and Dr. Josephson?s book puts it together and brings us up to date. ?Sinus Relief Now? is certainly a great reference for anyone with CAID problems. There are descriptions of just about every medication used in any of the treatments, both prescription and OTC. I learned several things I didn?t know before. For example, I didn?t know that antihistimines are only effective against allergies. I thought they would simply prevent excess mucus forming in any case. Also, Dr. Josephson feels that decongestants should not be used long term but only for a week at a time as he believes they can cause problems if used for long periods. In general the book is almost a reference work for any type of respiratory or digestive condition. New Study Finds Doctors Overprescribing Antibiotics

Doctors are unnecessarily prescribing antibiotics for sinus infections. According to the study published in the March issue of Archives of Otolaryngology-Head & Neck Surgery, US, antibiotics were prescribed for 82 percent of acute sinus infections and nearly 70 percent of chronic sinus infections.

This is a shocking finding as antibiotics can only kill bacteria and most sinus infections are caused by viruses, allergies or hormonal changes.

The study, by Hadley J. Sharp and colleagues at the University of Nebraska Medical Center, Omaha, US, used national data from 1999 and 2002 to determine the drugs that were prescribed for sinus infections by general practitioners, outpatient and emergency departments. The data came from 2 national surveys by the National Center of Health Statistics and was representative of the US population.

There are 2 types of sinus infections: acute and chronic. Acute sinusitis occurs for up to 4 weeks and is caused by infectious agents. Chronic sinusitis is usually defined as an infection with symptoms that are persistent for more than 12 weeks. It can be caused by allergies, hormonal changes or facial anatomy.

There are currently no effective drugs for chronic sinusitis. According to Dr. Donald A. Leopold, chairman of the department of otolaryngology at the University of Nebraska Medical Center, antibiotics are the best of a bad lot.

Many patients would call up their doctors and demand for specific antibiotics that may give them relief. They are aware of these drugs. They want prompt relief from the annoying symptoms of sinusitis. It's no wonder why doctors are giving in to their demands.

Although there are several tests to find out whether a sinus infection is caused by bacteria, they are often expensive and lengthy. The first test is an endoscopy, which involves inserting a small tube-like instrument into the nose to collect a sample of mucus from the sinus cavities. The second test is a nasal cytology, whereby a swab of mucus is taken from the lining of the nose to be examined. The third is taking an x-tray.

Even specialist doctors are not able to diagnose the specific cause without carrying out any tests. The symptoms of sinus infections are similar whether it's caused by bacteria, virus, allergy or other factors.

On the prevalent use of antibiotics, the researchers suggest that doctors could be treating secondary infections. Another possible explanation is doctors think that antibiotics work when their patients get better taking them. However, many infections resolve with or without treatment.

The researchers are concerned about the overuse of antibiotics giving rise to drug resistance and virulence of infectious bacteria. Although the public are aware of the increasing antibiotic resistance, many sinusitis sufferers are likely to continue demanding for antibiotics.

A diagnosis for sinus allergies can now be done by a blood test, which is much easier than the skin tests that were required in the past. In my case I had weak allergies to several things including dust, dust mites, and certain molds. Although my sinus allergies were not strong, my ENT specialist recommended that I start a series of injections to counter the above allergens (an allergen is an allergy causing substance, for example, dust or dust mites in my case). The doctor said that although he could not be sure if the shots would help much, we should proceed with them because we ought to try everything within our power to overcome my sinus allergies and sinusitis

I thus received shots weekly for about two years. For the second year it became inconvenient for me to go to the doctor?s office every week for a shot, so I was taught how to give myself the injections. This exaggerated response of the immune system can cause inflammation and result in the release of histamines and other chemicals in the body leading to the creation of excess mucus in the nasal system, then possible blockage and infection.

Common allergens causing sinus allergies include pollen from trees, grass and weeds, animal dander, feathers, dust, dust mites, molds, mildew, smoke, perfumes, cockroaches, industrial chemicals, insect stings and certain foods, such as milk, eggs, peanuts, shellfish, and chocolate, among others.

The best solution to avoid triggering sinus allergies is to avoid coming in contact with the specific allergen that affects the individual. As can be seen from the above list, however, that is often impossible. In addition to the injections discussed previously, one can also use nasal irrigation to clean out the nasal passages, and this will provide some help, even if it might be limited. Book Review of "Sinus Relief Now" by Dr. Jordan Josephson- Part 1

When I first started seeing an ENT specialist years ago for my sinus problems, I purchased a couple of books on the subject of sinusitis. When I mentioned this to my doctor and asked a couple of questions that showed a bit more knowledge of the subject than most people had, he frowned and had a worried look on his face. He then commented that having only ?a little bit of knowledge was dangerous?. I therefore appreciated the comments by Dr. Josephson in his new book entitled ?Sinus Relief Now- The Groundbreaking 5 Step Program for Sinus, Allergy, and Asthma Sufferers?. He stated: ?If your physician responds negatively to your newfound knowledge or is obviously uncomfortable working with a patient who is well educated about his or her disease, it?s another clear signal that it?s time to move on? (and find another doctor). I wish I had had this advice at that time, as the doctor eventually performed my first sinus surgery, caused me immense pain, and he didn?t improve my situation at all.

It should be mentioned at this point that Dr. Josephson is a well known and highly regarded ENT specialist and surgeon. He has several times been on New York magazine?s ?Best Doctors? list. This is an interesting concept and is explained clearly and thoroughly in the early chapters of the book. Dr. Josephson discusses the symptoms of all these maladies and then offers a CAID test, which helps the reader understand which branch of CAID is causing his problems. He explains symptoms and possible treatment options in great detail for the various ?limbs? of CAID, sinus disease, allergies, asthma, GERD and LPRD, and sleep disorders such as snoring and sleep apnea.

One point is made clear throughout the book: like high blood pressure, sinus disease cannot be ?cured? in the sense that it is gone once and for all, but it can only be controlled. But with this awareness and being knowledgeable about one?s problem, it is possible to control these health issues, and one can improve his or her life and not be forced to ?just live with it?.

There are a number of things I was pleased to see in ?Sinus Relief Now?:

1. Dr. Josephson is a sinus sufferer himself. He has undergone sinus surgery, uses the therapies he recommends, and ?knows the feeling?. I got the feeling when reading the book that this person has gone through some of the same pain I have experienced.

2. Dr. Josephson strongly emphasizes the use of nasal irrigation. He uses a sinus irrigation device every day, as do I. He also talks about using a neti pot for sinus irrigation, but I?ve used both and find the irrigator much more effective. I personally would not even recommend the neti pot method.

3. Dr. Josephson is a surgeon, and he pushes for a very conservative approach regarding turbinates. In the past many surgeons routinely cut out parts or even most of the turbinates in people. In recent years it is becoming apparent that this can have disastrous effects on patients over time, and there are many horror stories to read at the forum of the ENS Association web site. ENS stands for Empty Nose Syndrome, which is the term coined for this condition.

4. Dr. Josephson says that smokers with CAID problems simply must quit. He has some appreciation of the difficulty of quitting, however, and offers some guidelines for that as well.

If you've ever suffered from a sinus headache, you almost instantly recognize what it is as soon as it starts. You feel congested and stopped up. There is pressure in your head, your cheeks, and in, around, and behind your eyes. You nose often becomes stuffy, and it may even run. Another sinus headache!

But is it really a sinus headache? Are you sure? It could actually be a migraine headache, or even a tension headache. Sinus headaches are usually dull constant aches, whereas migraine headaches are usually described as throbbing pain, accompanied by nausea. Both headaches may make one sensitive to light and sound. Sinusitis can be caused by a variety of things, such as bacterial infections, fungal infections, immune deficiencies, nasal cavity structure problems, allergies, or even simple colds. All of these things cause changes in the pressure in the sinus cavities, which result in headaches.

People who have weak immune systems, allergies, asthma, or other conditions that affect the respiratory system are more apt to suffer from sinusitis, and therefore, sinus headaches. But some people who have none of these conditions may also develop sinusitis as well.

Most people who have frequent sinus headaches don't bother to consult with their doctors about the problem until it starts interfering in their day-to-day lives. The headache may go away, but the sinusitis that caused it will still be there, and the condition may worsen as time passes by.

I am one of the 35,000,000 Americans who suffer from chronic sinusitis. I?ve looked at numerous potential sinusitis cures over the years and have undergone two painful sinus operations. Bacteria can then invade the damaged areas and cause sinus infections. It therefore made sense that an option in the search for new sinusitis cures would be to treat the fungus rather than treat the bacteria with an antibiotic, which is often the therapy followed conventionally.

Mayo Clinic studies have shown that 75% of chronic sinusitis sufferers saw improvement when treated with a topical fungicide. The Therapy is called topical antifungal therapy. An antifungal called Amphotericin B has already been approved by the FDA for other treatments and is normally administered to patients as an injection. For sinusitis cures it is made into a topical nasal spray and applied daily. The recommended dose is 100 micrograms/ml. This medication is generally not available in most pharmacies today. One must get the prescription filled through a compounding pharmacy such as Anazao.
The best thing to do is find a physician who is trained in administering this therapy, and that doctor can no doubt explain how to obtain the medication prescribed.

In some people the eosinophiles do not react to fungi present in the nasal mucus, and these people generally do not have chronic sinusitis. Why the eosinophiles act differently in different people is not yet known. Also, it is difficult to administer a test which will tell if one person has this type of reaction occur in his body or not. However, this theory would explain why some people are chronic sinusitis sufferers even after extensive conventional treatments or sinus operations. The conventional therapies and surgeries to date have simply not addressed this issue, since it was not known prior to the Mayo Clinic studies and patient tests.

Sinus infection or sinusitis is inflammation of the sinuses. Sinuses are the hollow cavities in the bones around the nose. Mayo Clinic researchers have made great strides in recent years regarding sinusitis treatment and have come up with a new therapy for chronic sufferers called topical antifungal therapy. This particular sinusitis treatment is still new and is not widely practiced. This author, a sinusitis sufferer of long standing with two painful operations under his belt, has been in contact with one of the Mayo Clinic research physicians. I was pointed to several papers and articles describing the research completed thus far, the theory behind the research, and the resulting therapy.

In simple and general terms, the Mayo Clinic research showed that some people (i.e., chronic sinusitis sufferers), have a harmful immune reaction to fungi that others do not experience. The research demonstrated that fungi are present in the air and show up in the nasal mucus of just about everyone. In the noses of chronic sinus sufferers, it showed that certain types of white blood cells will attack the fungi that are present. In doing so these cells create a compound that damages nasal membranes. Once damaged, bacteria can easily enter and cause pain, inflammation and infection. Conventional sinusitis treatment often includes antibiotics to attack the bacteria. This new sinusitis treatment aims to attack the fungi instead, thus avoiding the nasal membrane damage in the first place. One drawback is that it is not easy to determine if a patient is someone whose white blood cells attack fungi in the nose or not. It is also not known why this white blood cell reaction occurs in some people and not in others.

Antifungals such as Amphotericin B and Itraconazole are used in this sinusitis treatment regimen. These have already been approved by the FDA for other uses, and they can be mixed by a compounding pharmacy such as Anazao to make the topical solution needed for this new therapy. Amphotericin B, for example, was only available in my local pharmacy as an injection medication. The pharmacist was not aware that it is sometimes reformulated as a topical spray for sinusitis treatment. Patients spray the antifungal into their nostrils daily. About 75% of the chronic sinus sufferers in one of the Mayo Clinic studies saw significant improvement in their conditions following this regimen.

I also contacted Accentia, the biopharmaceutical company who has obtained a license from the Mayo Foundation to produce and market medications based on the Mayo Clinic research. They informed me that they plan to market a product based on Amphotericin B, which will have the brand name SinuNase. They will start clinical trials soon, and I submitted my name as a possible participant. Apparently I would be a good candidate since I?ve had sinus surgeries that didn?t cure my problems.

Topical antifungal therapy is a new form of sinusitis treatment. It takes time for a new approach to become accepted in the medical community at large. I asked the Mayo research physician if he knew of a colleague in the Denver area, where I reside, who was utilizing this approach. He responded that he didn?t know anyone there, but gave me two names of physicians in Texas, which I also asked about. So it is obvious that this approach to sinusitis treatment is not yet mainstream therapy, but it does have momentum. Sick Sinus Syndrome affects on average three out of every 10,000 people in the United States. This condition becomes more common as we age.

Sick sinus syndrome is a type of bradycardia in which the sinus node or sinoartrial does not work properly. The sinus node is in the upper right chamber or the atrium of the heart; and is a small group of cells. This small group of cells contains cells commonly referred to as the pacemaker. The pacemaker cells generate the electrical signals that control the rhythm and pace of the heartbeat. These electrical signals travel from the sinus mode to the AV node. From here, the signal is sent along pathways that extend into the ventricles. This is what causes the ventricles to contract and pump blood into our lungs and throughout our bodies.

Sick Sinus Syndrome is not really a specific disease but is a collection of symptoms of signs that signify the SA node is not working properly.

Diagnosing Sick sinus syndrome can be very difficult since many people have no symptoms at all until it is in an advanced stage. The first symptom that most people recognize is a consistently slow heart rate or what is better known as bradycardia. The heart rate often falls to 40 to 50 beats per minute or even less before a person even realizes it.

Other symptoms of sick sinus syndrome include: * Fainting can occur when irregular heart rhythm affects the hearts ability to pump an adequate amount of blood to the brain. * Exhaustion and weakness can also be a sign that the heart is not pumping like it should. When the heart is not pumping an adequate amount of blood, the body sends the blood to the vital organs, thus allowing the blood flow to the muscles of the legs and arms to be inadequate. This can cause a person to feel weak, have less energy, and be unable to do many tasks that they once enjoyed due to tiredness. * Shortness of breath: Many patients with sick sinus syndrome describe their shortness of breath as not being able to take in enough air or they are just plain out of breath. * Chest pains, sometimes referred to as angina, can indicate that the heart is not getting enough blood. * Sleep apnea is a condition in which a person stops breathing in the middle of the night. Sleep apnea may be one of the contributing factors to Sick Sinus Syndrome since it reduces the amount of oxygen that is supplied to the heart. * Uncertainty or confusion is another symptom of this condition. In some cases, since the blood supply to the brain is inadequate, confusion and errors in judgment can occur. The individual may appear to seen sleepy, have problems with balance, or seem to be agitated or anxious.
